If this is your first visit, or you have not been seen at OSI in more than three years, complete these online before you arrive. It takes about 10 minutes, and your paperwork is ready the moment you walk in. Each one opens in our secure Updox patient portal, with no account or app to download.

Records & Authorization

Use this form to authorize the release or request of medical records, or to give us permission to share information with a family member, attorney, or another provider.

    Authorizes us to release your OSI records, or to request records from an outside provider on your behalf.

Records requests typically take 7 to 10 business days to fulfill. If you would rather send a signed paper form, fax it to (830) 624-7505 or bring it to your visit.

How to complete your forms

  1. Fill them out online

    Tap any “Complete online” button above. The form opens in our secure Updox portal, which works on your phone, tablet, or computer.

  2. Submit before your visit

    Finish the new patient forms and medical screening at least a day ahead so we can pre-register you. Your check-in will be much faster.

  3. Prefer paper?

    Call the scheduling team and we will mail a printed packet, or you can fill one out at the office. Fax completed paper forms to (830) 624-7505.

Need Help?

If a form will not open, a PDF is too large to print, or you would prefer a paper packet mailed to you, call the scheduling team. We can email a secure link or mail a printed packet to your address on file.
